VSA (Vital Signs Absent Response) Procedure

1. Medical Assist Report (MAR) to be completed whenever FIRE is FIRST on scene
   (KFD Defibrillator utilized or not).
2. Download MAR Form
3. Completed MAR emailed TO email DefibData and Stacey Hemstock
4. MAR to be documented in CRISYS as completed
5. Download guide for AED data
6. CRISYS: BVM and defibrillator pad usage
7. Replacing Equipment: replacing equipment process
8. Zoll Defibrillator Data Download: Download the data using non-rescue mode on defibrillator
   refer to the training bulletin

MVC (Motor Vehicle Collision) Reporting

1. MTO required on Conestoga Parkway / HWY 401 / on and off ramps
2. MTO needs to be documented in CRISYS benchmark
3. Refer to the reporting bulletin for details
4. MTO form
5. OPP Occurrence #, License Plates, Insurance Company, Policy Number information needed
6. Insurance Information mandatory when strikes a structure
7. City Vehicle involved in MVC requires PD, P/C, and Dispatch notification

• Lockbox and Public Education/Relations
• Not for Training, Pre-Incident Planning, HFS, In Service Inspection
